Self-Pay Rates
50 minute Individual Session: $175
Out-of-Network Benefits:
If your insurance plan has out-of-network benefits, you pay me directly and your insurer reimburses part of it later. I provide a superbill, which is an itemized receipt with the diagnostic and billing codes your insurer needs.
Sliding Scale
When I am able, I offer sliding scale rates. No proof of income or application is required.
Cancellation Policy
Appointments must be cancelled 24 hours in advance to avoid a late cancellation fee of $75.
If you are able to reschedule your appointment within the same Monday - Friday week, I will waive the late cancellation fee (otherwise the fee will apply).
No Show Policy:
If you’re running late for a session, I will wait for 15 minutes. If you haven’t arrived by 15 minutes into our scheduled session time, you will be charged a $75 no show fee or a no show fee equal to your current sliding scale rate (if we currently have a sliding scale agreement.) Please note, insurance does not cover no show or late cancel fees.
Good Faith Estimate
Under the law, health care providers need to give individuals who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
